Off Campus Registration Form

This form is used as part of the Internship Registration Process. It is completed and signed by the student who is planning to enroll in an internship or field experience for credit whether the site is located off or on campus. Submit this completed and signed form to your Faculty Internship Advisor for sign off and then scan and email the completed form to internships@smumn.edu.

Once all required paperwork and approvals have been received your internship will be approved and this form will go the the Registrar for enrollment in your internship credit. You may not begin your internship hours until your internship has been approved and you have been enrolled in the credits.
